Problem: Email functionality should be optional

Email is often a sticking point for new instance admins due to the difficulty of sending automated email. There should be a way to disable email confirmation, especially for instances aimed towards trusted members (e.g. friends and family).

Discussion:

• There is other email functionality, but that is of lower priority.
• Perhaps one simple alternative is to add tootctl accounts create into the GUI to let the admin create accounts manually. This will maintain the target use-case with friends and family.
• I'm not sure how this will work with sign-up approval; how do you notify the sign-up-ee that they have been approved? Perhaps that is not the target use-case.
