Creature equipped with Eater of Virtue doesn't get exiled when it dies

Eater of Virtue doesn't exile equipped creature on death. The trigger goes on the stack, but then nothing happens. It doesn't matter if it happened via combat damage or a removal spell, tested with both.

I'm still encountering this issue. Sometimes it was working during a cube draft, sometimes it wasn't. Here the Eater of Virtue triggers when the creature dies from Lightning Bolt, but the creature never actually gets exiled.
