"Used password field only and add OTP after the password" not working on OC 8

OC 8.0.2 OTP Master from 2015-04-19 OTP Settings: Authentication method: Two-factor authentication (User needs password AND OTP to connect, if user is in the OTP db file) Used password field only and add OTP after the password = on other = default

When trying to access with OTP enabled user, the following error is gotten: "ERROR: Authentication failed (and other possible unknown errors)". After 6 errors, OTP locked. OTP field never appears
