Request for notification of group approvals/denials

* Existing New Group Denied emails have a section for Organization Coordinator's Notes.  Talked to Chris and we would like to add a modal for New Group Denial and New Membership denials so that coordinators can state the reason for denying the request.
* Existing emails do not point back to the Personal Settings page.  Should the email notification of Group Approval / Denials be added to the Group Coordinator section?
* Should a notification be generated to indicate new group approval / denial?
