Plugin: Record ineffective execution of opportunistic mutations

Currently we only record effective opportunistic mutations, but this by itself is not enough to distinguish ineffective mutations from unreachable mutations. The latter should count as survivors while the former should not.

In the long term, the runner should gain logic to report the mutation coverage.
