Unknown Room disconnect Reason

My agent and my client begin a conversation, and after about 20 seconds, the logs appear as shown below.

I am sure that my client did not directly invoke the disconnect room, nor did the agent. How can I determine the reason? I checked the sessions in the web console, and everything looks fine.
