Duplicate memberships

There are users with multiple memberships in the same instance. This shouldn't be allowed.

As these memberships have all been created almost at the same time, I assume this is due to multiple clicks on the join button due to slow network connection or similar. I was able to reproduce that by doing a double click on join locally.
