Support file backed resolver

It would be nice to have linkerd-tcp run fully standalone and load all of the configuration settings / routing / etc.. from files.

Not a substitute for the real thing, but I've added a ./namerd.sh script that'll fetch/configure/run namerd with a file-backed namer.
