Allow to use an avatar when the camera is not active

When one attendee disables its camera, a pingoo appears instead. It would be nice to be able to specify an avatar. That could be specified before joining a conference when we specify a name).

The avatar location could even be stored as a cookie. Users could specify an image URL or a gravatar ID.

When the camera is active, no avatar option would be shown.
