[`Medicine.md`]: Figure 4-1: `med_open_airway.vd` does not exist.

In the wiki, the file Medicine.md has embedded a graphic med_open_airway.vd, but that file does not exist in a git checkout of the wiki:

Also, the corresponding rendered wiki page does not show a graphic at "Figre 4-1".

This currently makes automatic PDF-creation with wiki2pdf failing for me.

4.1 is not a standard JPEG image. It was vectorised into SVG code.

This wasn't initially clear though I found this out when I was developing my ePub version of the guide. So I took the code from the image, saved as a .svg and then converted back to a raster.

I have edited Medicine.md to now contain the correct image, and I have uploaded the image via the github function. But now the image is stored differently than all the other Images.

I don't know how to correctly upload the image as fig04-01.png to the wiki, please anyone who is able do it and fix the image inclusion in Medicine.md.
