Document how decisions are made in Design at Liferay

A foundational aspect of any organization is the process by which decisions are made and subsequent actions are taken — our handbook doesn't have this spelled out — we should at least have a brief outline of this, to answer questions like:

  1. What is our decision-making process?
  2. How is a consensus reached?
  3. How are disagreements handled?
  4. How are decisions appealed/contested?
  5. How/when are decisions changed?
