Minimize extraneous header includes

There's no doubt that GEOS has a lot of extra #include directives lying around. Some could be forward declarations. Others could just be not there at all! Finding them is a bit of a job, even with tooling, maybe because we just have so many. The tool that is mentioned most frequently is include-what-you-use.
