Clean up logic and documentation around <rules>, <onlyRunRules>, -Drules

It's hard to understand, from the code or the documentation, what the precedence is between these 3 ways of telling the plugin which rules to run. Some thought will need to be put into what the precedence should be, and then fixing the docs and code.
