Tru64 (5.1B-4, 4.0F, likely all versions) bus resets with IDE (CD-ROM, HDD, or both), boot fails

This issue seems to be the same as/related to #63, #8, and this very same ES40 bug from 2008, however on Tru64 this is triggered by simply trying to boot the install CD and the error is a lot more verbose. This happens even if I move the HDD or CD-ROM only over to SCSI while keeping the other drive on IDE.

Issue still occurs with Tru64 5.1B-6 + NHD7 + AXPBOX 1.1.0 SCSC CD-Rom + SCSI disk works single user boot skip installation. Full Boot install fails with filesystem bug mentioned in comments.

IDE CD Rom + IDE Disk works single user boot skip installation athough you get timeouts above. Full boot fails due to timoeuts. INSTALL fails due to timeouts

SCSI CD Rom + IDE Disk system core dumps.

UPDATE: Following configuration. CDROM IDE 0.0 DISK IDE 1.0

Timeouts occur less frequently.
