Document purpose of RenameThisFile.exe

It may very well be that I'm missing the obvious, but I don't see RenameThisFile.exe referenced anywhere in the code nor in the readme, so I think either an explanation or a removal of the file is sensible. (or both!)

RenameThisFile.exe is refferenced in the read me as a hyperlink "this", that leads to download of RenameThisFile.exe. It's purpose is to run in place of the original game exe file, such as DarkSoulsII.exe, which is what you should rename RenameThisFile.exe to, while renaming original DarkSoulsII.exe to game.exe.
