Upgrade to 1.0 migration, missing changes -> modifications rename

Hi,

The migration to upgrade to 1.0 miss the following column renaming :

rename_column :versions, :changes, :modifications

You might also consider updating the readme as so far it recommend using the fork from adamcooper
