Rating for each flashcard

Rating is now available on a course as a whole. Rating on each flashcard can provide feedback on how well it was "understood" by the user. This can be done during "learning" mode or "review" mode.

This could potentially be distracting during a review session. Enabling the rating can be disabled or enabled at the beginning of a review session.

I think chances are the learner wants to comment on a flashcard rather than rate it. E.g., if it's poorly understood, - they'd want to say what exactly is confusing, giving a bad rating won't be helpful to anyone. I think the comment section under the courses should be considered instead, there learners could discuss what's missing from this course.
