This happens to me since I started using background music a few years ago. I still haven't managed to consistently reproduce it but is related with my laptop going to sleep. However, I have a way to fix it consistently:

  1. Exit background music
  2. KillCore Audio Driver (Background Music Device.driver) (from the command line or activity monitor)
  3. Start again background music

It is annoying but at least I can use it.
