Add checklist and decision points

To make this a full-fledged one-stop-has-all-you-need kind of reference, there should probably be two additional sections:

Checklist For everything that is not requirements, but needs to be checked. Like..

• permalinks
• WP_DEBUG

Decision points For things that needs to be specified at some point between a client and a vendor (who has suggestions, I would hope). Maybe also include a suggested value. For stuff like:

• Max upload size
• Type of files to be uploaded
• Post revision retention period
