Make use of ajv useDefaults option for defaults, replacing openapi-default-setter

Same reasoning as #628, but a different option so I thought I'd make it a different issue. This would lighten the load on this project by making better leverage of ajv. It supports defaults in requestBody unlike this project. At least openapi-request-validator could support ajv options being passed in.
