It is a letter to letter conversion. That is the reason why it happened. Even I have found inconsistencies. പൈതലിനായ് pythalinaayu because മേഖലയാണ്‌ mekhalayaanu The ് gets converted to u.

An alternative is indic-trans, uses ML models for the transliteration. I haven't tried it, but I guess it should give better output for some cases.

This looks very interesting. As ml2en is a simple rule-based transliterator, it's very difficult for it to account for all possible edge cases.
