Can't get extension to work

I believe I've correctly installed the right software on my Windows PC and appropriate Chrome extensions. I've also installed StandardFirmata on my Arduino Uno and tested it with the "firmata_test.exe" (Firmata Test) program. However, when I run this extension, it always shows the Arduino as disconnected. What am I missing?
