Abilities/spells do not go to the targeted add

Hi, I hope this makes sense: What happens is that sometimes, when there is a pack of mobs and I cast an ability such as taunt or Moonfire, the ability does not go to the targeted add, it goes to some other add. Please let me know if you would like me to submit a video or if this is a known issue. Thank you,
