I run into this issue all the time as well. It seems when Sliding Panes is turned off it doesn't reset the pane width and positioning.
This is now available in Obsidian 0.16
You can achieve this via a CSS snippet See implementation of the font size here: https://github.com/kepano/obsidian-minimal/blob/master/src/scss/obsidian/fonts.scss#L17-L22
Have you explore Embedded Note Titles plugin? This may solve the first item on your list.

This is no longer relevant as right ribbon doesn't exist anymore in Obsidian 0.16+

Are you sure this is not happening in the default theme? This does not seem related to Minimal as far as I can tell.
You mean in focus mode?
