Lift the 254-character limit

The current 254-character limit on the size of the output seems arbitrary and not useful.

If the limit is to be kept (e.g. because it needs https://github.com/kelseyhightower/echo/issues/3 to be fixed to be lifted) then echo should exit non-zero when this limit is exceeded.
