Auto add custom FVTT permissions to custom-domains and remove permissions when domain is removed

There's no way to remove an authorization for Beyond20 to a specific site. When we request permissions for a FVTT site, we should auto-add it to the custom domains settings, and if a domain is removed from that list, we should automatically do a chrome.permissions.remove to remove the permission.
