Extended bolus doesn't add the extended part as a treatment just adds notes to the up front bolus amount. tconnectsync 0.5

It looks like the correct options to use for the Nightscout bolus treatment are setting an eventType of Combo Bolus, along with splitNow, splitExt, and duration fields.

I will look into making this change.
