Differentiate non-unique citation strings

If citation-js-utils generates the same string for two different references, MyST (probably with some help from citation-js-utils) should add a letter to differentiate them, e.g. Koch et al. (2012), Koch et al. (2012) -> Koch et al. (2012a,b).
