Simplify layout and IP selection

The host, port and layout can be changed by editing index.js, but that is not very accessible. A better way to do it could be a separate config file. Layout could also use the selected layout (in ProPresenter) in case the option is not set. The config file could also be accessible on the SD card on a Raspberry Pi, to simplify configuration.
