De-dupe new-account blocks before enqueuing

When stream.js starts up, it replays old @-replies to catch any it missed during a restart. If a new account @-replied many times in that window, it will trigger many actions, most of which will be resolved as duplicates. During @-reply replay we should de-dupe Actions before enqueueing.
