Default is in the url but 404 error

Hi,

i have installed your plugin. The problem is in fronted have every url the /default/ in the url but give a 404. How i can delete the "/default/" in the url? without default is the plugin working

I can confirm that this happens for URLs of type "Custom" (as opposed to "System") in the "URL Rewrite Management" section of the admin.
