After the first scene, when you transition to new scenes it repeatedly plays the animation of coming back to life. This animation should only play after a death scene, when starting a scene again.
In the beginning, after crossing the bridge (sword then up) it will show Dirk run into the castle.
The next scene is a room full of doors. After dodging the black stuff and going right it will randomly choose between one of three different rooms. At this point, it begins playing the animation of Dirk coming back to life every time you make it to a new room, whether you die or not.

Thanks eXo for giving this a test! 😄 I'll double-check the behavior on my HW Maxima setup with DOSBox staging one of these days when I have some free cycles. I don't recall the details of this when I tested this last year.
