sample app not running out of box on Evolutions config

applyEvolutions.default=true needs to be set in application.conf to get sql script to be executed

Initially I thought it would be good to have production-like defaults defined, but seeing as most people only use it as a skeleton anyway, I think we could add that - would you mind opening a pull request please?
