document format for CSV file

The README mentions the need for a CSV file, but it would be useful to add an example to show the exact format for this file.

Salut @fmichonneau, I have made this change but only incorporated it into the branch dev. It will be folded into master soon. In the meantime, if you move to the dev branch, it is incorporated along with tests to ensure the entire analysis works as expected (by using ./banzai.sh test).

Hey @fmichonneau, if you ever tested this, did you find the example and description that I added to be sufficient? Happy to take more input. I think in the end it would be best to link to a google sheets template, which the user can then download as CSV with more predictable results than CSV export from Excel.
