Comments losing their position

This elusive bug is still around. I don't know how to reproduce it, but we need to find out and fix.

Hypotheses:

  • adding text after a inline comment causes comments to shift
  • refreshing before save complete
  • affects content after a code block?

this also happens when the editor fails to save... see in this GIF below... in the bottom right hand corner it shows that it last saved 2 minutes ago. that means it didn't successfully save recent changes.
