Improve clean_changesets()

A couple of improvements:

  1. Convert all changeset links to a link to the original bitbucket url for the changeset. Example: See https://bitbucket.org/hpk42/pytest/changeset/9658138fc210bcc72a5fcc215eaa4e01f991e837 for details.
  2. Also convert formatting like [changeset:2320](changeset:2320)
