Make require name, gem name, and github project name consistent

Right now, the gem name is salesforcebulk, but the require file name and the github project name are both salesforce_bulk. You should consolidate on a single name and use it everywhere.

You might even want to completely change the name so that users don't get confused with jorgevaldivia's salesforce_bulk.
