Add General Equivalence Mappings for conversion of ICD-9 to ICD-10 codes

Where's a good start for this and - "icd does not covert ICD-9 to ICD-10 codes yet" Is it simply update the GEMS to the project and write the function to do the look-ups? Are these the most recent GEMS?
