Allow defaults via provider

Looks like alot has changed, but I've pretty much just put everything that was there in the this.$get property of the provider as opposed to using the shortcut factory method. This allows me to expose the setDefaults() function on the provider.

Mostly useful to be able to set the default path or expires options so you don't have to provide them on every call to ipCookie. Didn't include the minfied file as I assume you'd want to change version numbers etc.
