Feature Request - Max Cost Per Minute and Max Connection Fee

On some other platforms I have seen a 'Max Cost Per Minute' and 'Max Connection Charge'

These would be great additions to Ivozprovider.

At a client level there are two additional fields for these.

Before the call is authorized, each field is checked to see if the cost of the call would exceed either the max connection charge or the max cost per minute and if either is exceeded, reject the call.

This helps hugely the fight against fraud and also helps the client with preventing high cost destinations being dialed.
