Add Issue Template

I see many repos including some Intuit repos like https://github.com/intuit/Tank, https://github.com/intuit/Traverser and https://github.com/intuit/user-data-for-fraud-prevention have an issue template to report bugs and request features. I believe this will be useful here as well.
