Add domainname uniqueness constraint to whois table

Set uniqueness and not null constraint to whois_records table column name in the registry database.

Auditing the whois data recently revealed multiple domains with two records in the table and on with no name set at all.

There cannot be multiple records for a sinlge domain name among the active records at any time.
