Add CLA/Contributing document

Per Intel guidelines we don't need CLA, but we require use of the Developer Certificate of Origin (DCO)/“signed-off-by” convention -- we need CONTRIBUTING and CODE_OF_CONDUCT files that explain our contribution and community behavior requirements --- see here for an example: https://github.com/intel/amber-docs
