Why `survival` package does not have `::` calls to functions?

I noticed after a simple Surv function search that there are many missing references to the package survival. I think we should fix it. See #750 for an example where the output actually changes if the package is explicitly referenced (that could be also an error on survival side)

i am thinking of decoupling this. this issue requires a bit more discussion and planning. let's put it back into the backlog, and revisit in the next PI
