I'm trying to imagine what an unrelated alternate label would be. Or is this supposed to mean 'alternate label of a related entity', in which case it should be named such.

I think this is this typical categorisation of mappings in EXACT, BROAD, NARROW, CLOSE and RELATED. This "related" here comes from that categorisation, and their related roughly translates to "analogous but in a different branch of the ontology". I find these a bit useless, but they are widely used, for example:

BAF43 is a related synonym of "whatever the protein is that is expressed by the gene named BAF43".
