do not prefix heading links with `user-content`

let's actually make prefixheadingIds false by default.

Careful, potentially dangerous (not sure if you knew that already?).

I had some similar discussions recently. And opted not to drop the prefix. GitHub still prefixes their headings, and implements their own scrolling to those headers (I think?). Related: https://github.com/jch/html-pipeline/pull/111
