Fix file path

The require in index.js wasn't pointing to the plugin file. This PR fixes it.

Ping! It'd be great if this were resolved!

For the moment, I'm working around it by vendoring it locally, but having it be fixed here would be great!

@johanbrook could you merge this PR and publish new version to NPM? This plugin is currently broken and this PR would fix it.
