I did not see the small boxes.

I cannot adjust the position of the input test box.

These information are automatically filled when the related information are set in your profile aka in your "settings" (see screenshot).

I'm agree, it isn't maybe straight forward but I didn't found a cool valuable UX, to explain that easily without over engineering the thing, any better idea?

Have some defaults for empty values e.g. FixMeNow at GitHub.com etc. Add a note to direct to settings as needed.

Not a bad idea, like a css ::after if all empty or something, will think about it 👍
