The icon in the Dock is owned by Terminal.app. TotalTerminal.app does not run, it is just a launcher which does what would SIMBL do: it injects some code into Terminal.app....

Please check out if you don't have any other custom scripting additions in: `/Library/ScriptingAdditions` and `~/Library/ScriptingAdditions` They may interfere with TotalTerminal.osax. Also try to change your keyboard shortcut.
