Bug Description

Create a case to any disease. Go to Symptoms tab. Set any symptom as YES. First Symptom and Date of Symptom Onset fields will be enable. Save the form.

Reset the symptom as NO or UNKNOWN

First symptom is cleared its value but not the date of symptom onset.

Save the symptoms details.

Now this date of symptom onset value is saving(which should not).

If the created case has samples, the same value (date of symptom onset) is displaying(which is obviously wrong)

Reproduction Steps

1. Create a case with any disease(eg, AFP)
2. Go to Symptoms tab and select any symptom as YES.
3. First symptom and Date of Symptom Onset two fields will be enabled.
4. Enter any Date of Symptom Onset date.
5. Revert the symptom to NO or UNKNOWN
6. Now first symptom drop down value is cleared but not the date of symptom onset.
7. Save the symptoms form. This date is saving in the database.
8. Create one new sample for that case(existing sample of the case as well)
9. Open the sample
10. Check the Date of Symptom Onset field in the Case card of the selected symptom.
11. Saved value is showing(actually, the case has no symptom at all, because it was reverted). ...
